You are asked to submit certified copies of the following documents via email.

1. 2 passport size photographs 
2. Birth Certificate
3. Academic qualifications (CSEC, CAPE, O' Level, A' Level)
4. Academic/ Theology Certificate, Diploma, Degree
5. Transcripts from Tertiary Institution

Please note that documents must be certified by a Notary Public, JP, Attorney at Law or Minister of Religion.
